DENTAL BENEFITS

· ORAL EXAMINATION/DIAGNOSIS AS NEEDED

All the instructions and commission fees for the bank wires can be found within the account management system, in the Menu > Load Account > Wire Transfer

· ORAL PROPHYLAXIS EVERY SIX (6) MONTHS (TWICE A YEAR)

· GUM PROBLEM CONSULTATION AND TREATMENT PLANNING

Please note that a bank wire takes up to 5 working days to reach us. If you see your balance unchanged after 5 days, please send us a request to chase the payment. Full payment details will help us to identify your deposit and the reason for delay.

If you are not a Polaris customer and you have wired funds to one of our customers, who provided you with our bank details, then please contact the intended receiver of the deposit to get the status update of the wire. Polaris does not disclose financial information to 3rd parties.
If, for some reason, your contractor is not providing you with updates on the wire, please fill in our payment search request form and we will get in touch with the intended receiver.

· SIMPLE TOOTH EXTRACTION WHEN INDICATED (EXCLUDING IMPACTION)

Please, send the payment amendment form to your bank /sender bank. There is no expiry date on such requests. If the amount has not been refunded to the sender, the information can be updated with the destination bank and the funds delivered to the receiver.
Contact the receiver (your contractor), requesting them to chase the deposit. It is necessary to get a SWIFT-confirmation from the sender bank.

· TEMPORARY FILLINGS WHEN INDICATED

Polaris does not disclose financial information to 3rd parties. Contact the intended receiver of the deposit (your contractor) or the sender bank to get the status update of the wire. If your bank refuses to amend payment details, you can recall the deposit through them.

· RE-CEMENTATION OF LOOSE JACKET CROWNS

In order to send money you need to specify recipient’s first and last name and his/her mobile phone in an international format. At the branch you need to prove your identity with a valid ID (passport, permanent residence card). For sending money online, you have to go through verification process first. For more information on “How do I get verified” please see Account Section in FAQ.

Polaris offers an opportunity to send the money transfer from the bank account. Same documents are necessary to be able to receive a transfer to a bank account. Verification requires you to upload color copies of the following documents

  • Passport (ID). The copy must feature document number, photograph, full name, date of issue, residential address, and expiry date.

  • Any other identifying document (driving license, copy of insurance card). The copy must feature the document number, photograph (if available), full name, date of issue, date of expiry.

If you want to make transfers using the Internet-banking and receive the transfers directly to your bank account it is also necessary to present the copy of a bank statement at one of our offices or upload it online.

· ADJUSTMENT OF DENTURES

When making an international wire transfer you can usually choose between different options, depending on who should pay the fees

  • OUR instruction means you as the sender pay all transfer charges.

  • SHA (shared) means you only pay your bank's outgoing transfer charge. The recipient pays his charges.

  • BEN (beneficiary) means you do not pay any charge, all will be paid by the beneficiary.
